The upcoming Medicaid requirement changes, explained

To our freelancer community —

When the “One Big, Beautiful Bill Act” passed in 2025, it cut $1 trillion in federal healthcare spending and allocated the money instead to national defense and Immigrations and Customs Enforcement. Although the bill became law over one year ago, the changes to our healthcare created by the bill are still being rolled out now.

Beginning January 1, 2027, there will be new requirementsfor how Medicaid participants get and keep their health coverage. We know that many freelancers rely on Medicaid to access healthcare, and that this provides yet another challenge — in a series of challenges — for freelancers to get even basic medical coverage.

If you are on Medicaid, read our blog to familiarize yourself with the changes coming. And if you are concerned about these changes and want us to know how they will affect you, email us at community@freelancersunion.org.
